Children are not permitted to carry medication including cough drops. ALL MEDICATION must be administered from the clinic. Parents must deliver the medication to the clinic and bring only the necessary amount for school. ALL medication must be in the original container and cannot be expired.
A parent or guardian on the student’s emergency contact list may check in at the front office and request to administer medication to their child. The child cannot take medication back to class with them.
Over the counter medication for less than 6 weeks must be accompanied by a note from the parent authorizing administration of the medication, reason and dose, time and amount. Dosages cannot exceed the recommendations on the medication packaging.
Prescription medications require a physician order (see form below.) All prescription medication must be in the original pharmacy container with the child’s name, medication name, and dosing instructions. Daily medications must have a specified time of administration included on the physician order form. A second bottle from the pharmacy can be requested from the pharmacy by the parent for the school clinic. Medications such as antibiotics for 3 times a day or every 8 hours are to be given at home. Just follow a before breakfast, after school and before bedtime schedule.
Any unused medication not picked up by a parent or designated guardian will be destroyed.

For food allergies and other life-threatening allergies, provide written medical documentation, instructions, and medications as directed by a physician, using the Life-Threatening Allergy Action Plan as a guide. Include a photo of the child on the form.
Epinephrine or other allergy medications listed on the action plan can be kept in the clinic. For children who carry their own anaphylaxis medication, please complete the Permission Form for Self-Administration of Asthma or Anaphylaxis.
If your child has asthma, please complete the Asthma Action Plan Form. Inhalers can be kept in the clinic with this form. For children who carry their own inhaler, complete the Permission Form for Self-Administration of Asthma or Anaphylaxis.

Texas Department of Health requires all school age children to be up to date on immunizations. Children are excluded from school until the required immunizations are obtained and a copy is given to the school nurse.
For an elevator pass or a pass to leave class 5 minutes early due to injury, please provide a doctor’s note specifying the necessary accommodation and how long it will be needed. 5-minute passes will allow the student to leave class 5 minutes early but will not excuse tardies.
If your child has a history of seizures, please complete the Seizure Management and Treatment Form. Contact the nurse regarding what type of seizure your child has, when their last seizure was, and any rescue medication that they are prescribed. Rescue medication can be kept in the clinic in case of a prolonged seizure at school.
If your child has diabetes, please contact the school nurse. You will need to provide a current diabetic care plan from your child’s doctor. All diabetic supplies must be provided by the parent.
